We are best suited for generally healthy individuals or those managing three or fewer chronic conditions.
This Allows us to focus on keeping you healthy, Rather Than Just Managing Complex Illness.
Monthly Membership Rates
A One-time Registration FEE of $100 Applies to all New Members.

Note: Membership enrollment is currently capped at age 60 for new patients.

Two Dedicated Wellness Visits
We proactively manage your health with two scheduled checkpoints per year
- Annual In-Person Physical: A comprehensive wellness visit including physical exam, EKG, and review
- 6-Month Wellness Check-in: A scheduled telemedicine visit to review your progress halfway through the membership year.
Urgent Care & Access
- Direct Access: Reach your provider via cell phone, secure text, or email
- 3 Free "Urgent Care" Telemed Visits: Included annually for unexpected illness (in addition to your scheduled wellness visits).
- Priority Scheduling: Same-day or next-day appointments are usually available for established patients

Patient Eligibility Our Limited Primary Care model is designed for patients
between the ages of 7 and 60 with three or fewer chronic conditions. This
ensures we can dedicate the necessary time and resources to every member.
Medication & Treatment Exclusions We prioritize safety and evidence-based
wellness. Therefore, our practice does not prescribe or manage:
- Narcotics / Controlled Substances (for chronic pain management)
- Blood Thinners / Anticoagulants (e.g., Warfarin/Coumadin)
